Every day we experience the frustration of patients being unable to communicate with their mail order pharmacy, unable to get their medications on time or worse yet, they get buried with drugs they don’t need. According to mail order pharmacies, in an effort to promote medication compliance, they offer to automatically ship a 90 day’s supply of all of a patient’s medications every 3 months, whether they need them or not. Patients end up bringing the “extra” medication to their local pharmacy because they don’t know what to do with it or how to stop the shipments.
As a local community pharmacist, I find this whole process appalling. To see so much drug wasted, when so many people cannot afford to get the medications they need. Of course, we make every effort to get these “extra” medications to our free clinics, but most times they are completely unusable, as in the case of $2,000 worth of insulin that got delivered to one of our patients while she was away on vacation. She alerted her mail order before her trip, yet they shipped it anyway. The insulin sat on her porch in the hot JULY sun for over a week! Her insurance company replaced it for her at no charge, but who do you think covered that loss?
What if the patient has passed away? What if the patient no longer needs the medication or the drug has been changed? Do you think the mail order pharmacy takes a loss on the drug waste that gets auto-shipped? I’m certain these medications are billed to the health plan. All those expenses ultimately get factored into health insurance premium prices. That means we foot the bill!

I thought about back when I decided to apply to pharmacy school. I thought about the motivations and thoughts going through my head as I made my final decision. Some of those same thoughts have recently returned.
Back when I was debating my future, several things impacted my final decision. I wanted a stable career that would allow me to make a comfortable living. Helping people was also something that drew me to pharmacy. And a healthcare related job was always appealing to me.
But there was something else that drew me to pharmacy. I thought the profession of pharmacy gave me an opportunity to be a part of something bigger than myself. And that meant a lot to me.
It was that idea that stuck with me over the years. Being part of a huge profession of individuals whose mission was improving health was a big plus for me. It made being a pharmacist more than just a job. It was a profession.
Now I feel that same sense of opportunity in front of me. The profession of pharmacy is at a critical crossroads. The next couple of years may shape our profession for decades to come.
Pharmacists have a rare chance to fundamentally change and define our roles. Healthcare is an evolving field. This means pharmacists can adapt to any changes that come or risk being left behind.
We often discuss the dispensing function of pharmacists. It’s a point of concern for some pharmacists that our impact on healthcare is often limited by our involvement in the drug delivery process of medicine. Sure, there is more to pharmacy than dispensing. But many positions within the profession do seem to be self limiting.

And that brings me back to that idea I had before becoming a pharmacist. I wanted to be a part of something bigger than myself. And these crossroads we now face as a profession gives us another opportunity to do just that.
This is a rare chance for pharmacy to help secure a vital role in healthcare instead of being relegated to some volume based repetitive motion job. This is a chance to secure our futures and livelihood. This is a chance to be part of something bigger than ourselves.

Since the mid-1970s, the puppeteers have been the guys who have taken our profession to the brink. The non-pharmacist MBA bean-counting Masters of the Universe. They have NO standing in pharmacy. They have never acted as a cashier. They have never accepted a new prescription. They have no clue how to enter a new patient, new insurance and then type the prescription. The idea that they would have any sense of what the pharmacist does is laughable. I’d love to see one of these Masters of the Universe at the drive-through. How would he handle a guy smoking cigarettes and demanding his Norcos now. His 20 year old Cadillac is spewing fumes through the drive-through drawer and he refuses to turn his engine off. His obese girlfriend with a tattoo of a Big Mac on her neck is eating “healthy” food. A deep-fried chicken sandwich.

Our industry is on the brink of failure. We have been trying to maintain a 50 year old model without the profit. The model is not sustainable.
When our industry is unable to deliver the prescriptions that our culture demands, turn out the lights. They are watching us. Who are “They”? The interests that want dispensaries and not pharmacies. A lot more mail order. Central filling. This is a horror to think about.
10,000 Americans turn 65 every single day. Our population is 330,000,000+. Our culture expects prescriptions when they go to the doctor. What can you give me, Doctor? is the question. Big Pharma will answer that question.
The Masters of the Universe have an answer for diminishing returns. Well, Larry and the rest of you executives enjoying this catered lunch of Rhode Island clams, I know what we can do. I am an MBA and I know that the answer is to cut overhead. Turning off half the lights is so 1970s. How about we cut the budget for ancillary help. They don’t need a cashier after 7:00PM. (One again, the fucker has never come within a football field of the working side of a drive-through). They don’t need to have technician overlap on Tuesday, Wednesday and Thursday. This would save us about $400.00 a week. That is pure profit. (I like calling the Masters of the Universe FUCKERS because they have been sticking it to us for four decades) Larry hands out big cigars and the MBA Master of the Universe thinks What a good boy am I.
By the way, you are not going to find MBAs who were “A” students at The Wharton School working for CVS. You will probably find “C” students from a public school at best. Night school at worst. The brightest MBAs from Stanford are on Wall Street getting huge bonuses from moves that lost billions. Go figure.
Having non-pharmacists design the pharmacy business plan is like having an MBA running a medical office. Doctors are not stupid. The people who have been running drug store companies make you wonder.
Okay, Plagakis, get to the point. If our industry was still a 40% business, we would have no problem delivering the crush of prescriptions to come. We could have extra cashiers, extra technicians. The technicians beyond the RPh:Tech ratio could be doing maintenance work. We could run streamlined operations and still make a terrific profit. We could counsel.

JP… you forgot one thing… the PBM’s started out with a 40% margin.. and our industry DISCOUNTED the copays.. so they lowered the fees until we stopped discounting the copays… the more efficient we became – computers.. the fees kept falling… the MBA’s didn’t want to say NO to that growing chunk of business.. so they kept accepting the lowering fees… at some point the system will implode..from trying to increase the “efficiency” of the Rx dept…until one day it will collapse under its own weight/volume per manhour.
